Health History & Authorization – REQUIRED – this can be printed and completed OR filled out online by logging into your account
Physical Exam Form – REQUIRED – This form must be signed and completed by a licensed physician. A physical must have been performed within 1 year of the camper’s arrival date at camp. A NY State physical form is accepted.
Standing Orders – REQUIRED – This form must be signed and completed by a licensed physician. The Standing Orders form is required if any medication is to be given at camp, prescription or over-the-counter, including over-the-counter medications that may need to be provided by the camp health director in the case of minor injuries or ailments.
Camper & Family Get Acquainted Form
Some of our programs include activities that take place outside of the camp property, referred to as “offsite trips.” These programs require additional permission forms. All 2026 programs with offsite trips are listed below, with the relevant forms linked.
